If you would like to register by mail, contact our office and request a registration packet. It may take two to three weeks to fully complete the registration process. You may also download the three required registration materials: 1. Program Registration Form (found at the end of this document), 2. Confidential Health Questionnaire (found at the Health Questionnaire link), 3. The Program Agreement, Release of Liability, and Assumption of Risk form (found at the Liability Form link).
After you’ve filled out the forms, you’ll need to have The Program Agreement, Release of Liability, and Assumption of Risk form (Liability Form) signed in the presence of a legally authorized witness (Notary Public), and then get a copy made of your insurance card. If you don’t have health insurance, please include a note stating that you do not have health insurance. Once you are truly committed to going, you may send in your deposit with the completed forms. You will become officially registered when we receive your deposit. We recommend that you give yourself plenty of time for preparation.
All Sacred Passage™ and The Way of Nature™ programs require a 50% deposit. Final payment is due 4 weeks prior to the start of the program. Please make out all checks to: John P. Milton
Our deposit policy is as follows:
- After receipt, deposits may be non-refundable at any time.
- For cancellation within 4-8 weeks of the start of the program, a 50% refund is given.
- For cancellations within 4 weeks of the start of the program, no refund is available (in the case of a cancellation due to a health related emergency, the deposit may be transferred to another program).
- We will provide a full refund if we cancel the program.
